What to bring

If you are a patient visiting OhioHealth Primary Care Physicians for the first time, please bring with you:
 

  • Insurance card
  • Driver’s license
  • List of medications
  • Names and telephone numbers of prior physicians


We request all our patients to bring their insurance cards with them to every visit, as well as their co-payments.


If you are a new patient, please complete this New Patient Registration Form and bring it with you to your first visit.


What to know

  • If you are having blood drawn at our office, you’ll be required to fast 8 hours before the blood draw.
  • If you must cancel your appointment, we request 24-hour advance notice. Cancellations made less than 24 hours in advance are subject to a fee.
  • Prescription refills require 48 hours notice.
  • Long term pain control services with narcotics are not provided – patients will be referred to a pain specialist.
  • Co-payments are due at time of service.

For family and caregivers

  • Children under 18 should be accompanied by a parent or an adult with a letter of parental consent for treatment.
  • Caregivers of forgetful or mentally challenged patients should plan to attend office visits with the patient.
